Overview

**Coxsackievirus A21 + mitomycin C** is an investigational combination therapy under evaluation for the treatment of non-muscle invasive bladder cancer (NMIBC). *Coxsackievirus A21* (CVA21), marketed as Cavatak, is an oncolytic virus that targets and lyses cancer cells expressing ICAM-1 (intercellular adhesion molecule 1) and decay-accelerating factor (DAF), leading to direct tumor cell killing and potentially enhancing antitumor immunity. *Mitomycin C* (MMC) is an antineoplastic antibiotic and alkylating agent that functions primarily by cross-linking DNA and inhibiting its synthesis. When used together, low-dose mitomycin C increases surface expression of ICAM-1 on tumor cells, significantly enhancing the ability of CVA21 to infect, replicate within, and destroy those tumor cells. This synergistic effect has been demonstrated both in cell lines and ex vivo tumor samples, supporting ongoing clinical trial development for this combination in NMIBC[1][3][5][7][9].
