CPX-351 + glasdegib is a combination regimen under investigation for the treatment of acute myeloid leukemia (AML), especially therapy-related AML and AML with myelodysplasia-related changes. CPX-351 is a liposomal encapsulation of cytarabine and daunorubicin in a 5:1 molar ratio, designed to enhance the synergistic cytotoxic effect and improve drug delivery to leukemic cells. Glasdegib is a small molecule inhibitor of the smoothened (SMO) receptor, a component of the Hedgehog signaling pathway, which is implicated in leukemia stem cell survival and resistance to chemotherapy. The combination aims to enhance anti-leukemic activity by combining intensive chemotherapy with targeted inhibition of leukemia stem cell survival, potentially improving outcomes in high-risk AML subgroups.
