Overview

This therapeutic protocol involves the sequential administration of CPX-351 (Vyxeos) as salvage chemotherapy followed by a specialized allogeneic stem cell transplantation (SCT) known as a haplo-cord graft. CPX-351 is a liposomal formulation containing cytarabine and daunorubicin in a fixed 5:1 molar ratio, designed to optimize synergistic antileukemic activity while reducing systemic toxicity. The haplo-cord transplant procedure, pioneered at institutions including Weill Medical College of Cornell University, combines CD34-selected hematopoietic progenitor cells from a haploidentical (half-matched) family donor with an umbilical cord blood unit. This dual-graft approach aims to provide rapid initial engraftment from the haploidentical cells while the cord blood unit provides long-term hematopoiesis and a potent graft-versus-leukemia effect. The protocol is primarily investigated for adult patients with relapsed or refractory acute myeloid leukemia (AML) or myelodysplastic syndromes (MDS).
