Overview

**cretostimogene grenadenorepvec + gemcitabine** is an investigational combination therapy for high-risk, bacillus Calmette-Guérin (BCG)-unresponsive non-muscle invasive bladder cancer (NMIBC). Cretostimogene grenadenorepvec (CG0070) is a conditionally replicating, recombinant oncolytic adenovirus engineered with the E2F1 promoter for selective replication in tumor cells with retinoblastoma (RB) pathway deficiency. Upon intravesical administration, cretostimogene infects and replicates in RB-deficient bladder cancer cells, resulting in cancer cell lysis and release of tumor-specific antigens and viral particles. The virus is further engineered to express granulocyte-macrophage colony-stimulating factor (GM-CSF), amplifying the local and systemic antitumor immune response by recruiting and activating dendritic cells and cytotoxic T cells[1][2][4][6][8]. Gemcitabine is a nucleoside analog antineoplastic agent that inhibits DNA synthesis and promotes apoptosis in dividing cells, commonly used in bladder cancer as intravesical chemotherapy. In this combination, cretostimogene provides tumor-selective oncolysis and immune stimulation, while gemcitabine provides direct cytotoxicity against proliferative cancer cells. The combination aims to improve complete response rates and durability for patients ineligible for (or unwilling to undergo) radical cystectomy.
