CRS-207 + epacadostat is an investigational combination therapy developed for use in cancer, notably platinum-resistant ovarian cancer and metastatic pancreatic cancer. CRS-207 is a live-attenuated, double-deleted *Listeria monocytogenes*–based immunotherapy engineered to express mesothelin, which stimulates a tumor-targeted immune response. Epacadostat is a selective small molecule inhibitor of indoleamine 2,3-dioxygenase 1 (IDO1), an intracellular enzyme that suppresses immune responses by catalyzing tryptophan degradation to kynurenine; inhibition of IDO1 reduces immunosuppression in the tumor microenvironment[5][7]. The combination is designed to enhance antitumor immunity by simultaneously activating immune recognition (CRS-207) and decreasing tumor-induced immunosuppression (epacadostat)[1][5]. Trials have assessed safety, tolerability, and preliminary efficacy primarily in platinum-resistant ovarian, fallopian tube, peritoneal cancer, and metastatic pancreatic cancer[1][2][5].
