CRS-HIPEC + LSTA1 is an investigational combination therapy being evaluated for the treatment of peritoneal metastases from colorectal, ovarian, and appendiceal cancers. The regimen combines cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) with LSTA1 (formerly CEND-1), a cyclic iRGD peptide. LSTA1 is designed to enhance the penetration of co-administered chemotherapy into solid tumors by activating a trans-tissue transport pathway. It first binds to alpha-v integrins (specifically alpha-v beta-3 and alpha-v beta-5) on the tumor endothelium and is then cleaved to expose a C-end Rule (CendR) motif that binds to neuropilin-1 (NRP-1). This binding triggers the transport of chemotherapy through the tumor stroma, potentially improving the efficacy of HIPEC in treating peritoneal surface malignancies. The University of California, San Diego is currently conducting a Phase I trial to assess the safety and efficacy of this intraperitoneal combination.
