Cryotherapy + imiquimod is a combination therapy used primarily for the treatment of various cutaneous lesions and skin cancers such as actinic keratosis, superficial basal cell carcinoma (BCC), Bowen’s disease (BD), penile intraepithelial neoplasia (PIN), and limited cutaneous Kaposi sarcoma. Cryotherapy works by inducing cellular stress and apoptosis through rapid freezing of tissue, leading to direct cytodestruction. Imiquimod is an immune response modifier that acts as an agonist at toll-like receptor 7 (TLR7), stimulating innate immunity and promoting the production of interferon-alpha, interleukin-1 alpha, and interleukin-12. The combination leverages the immediate cytotoxic effects of cryotherapy with the immunomodulatory field effect of imiquimod to maximize lesion clearance—including subclinical lesions—while minimizing systemic toxicity. This approach is minimally invasive and has shown efficacy in both monotherapies but appears synergistic when combined[2][3][5][7]. The regimen may vary but often involves initial lesion-directed cryosurgery followed by topical application of imiquimod cream for several weeks.
