CS-1008 + irinotecan is an experimental combination therapy evaluated for the treatment of metastatic colorectal carcinoma in patients who have failed first-line oxaliplatin-based chemotherapy. CS-1008, also known as tigatuzumab, is a humanized monoclonal antibody that targets and binds to death receptor 5 (DR5, also known as tumor necrosis factor-related apoptosis-inducing ligand receptor 2, or TRAIL-R2) on the surface of cancer cells, inducing apoptosis via activation of the caspase cascade. Irinotecan is a topoisomerase I inhibitor that interferes with DNA replication in cancer cells. Preclinical rationale and early clinical data suggest that irinotecan upregulates DR5 on cancer cell surfaces, potentially enhancing the efficacy of CS-1008. This combination was studied in phase 2 randomized clinical trials to assess its safety and efficacy compared with irinotecan alone, with the aim of improving progression-free survival in metastatic colorectal cancer patients who had failed prior chemotherapy[1][2][3][6].
