Overview

This is a combination therapy consisting of three components for the management of diabetes, typically type 2 diabetes mellitus: - **Continuous subcutaneous insulin infusion (CSII)**: This involves the use of an external, programmable insulin pump that delivers rapid-acting insulin subcutaneously in a continuous basal rate, with bolus doses given at meal times. CSII mimics physiological insulin delivery and aims to optimize glycemic control, reducing glycemic excursions and HbA1c levels compared to multiple daily injections[2][4][6][8][9]. - **Metformin**: An oral biguanide antihyperglycemic agent that decreases hepatic glucose production and increases peripheral insulin sensitivity, particularly in muscle tissue. It is a first-line treatment for type 2 diabetes and provides some cardiovascular protection[3]. - **Pioglitazone**: An oral thiazolidinedione that improves insulin sensitivity by activating peroxisome proliferator-activated receptor gamma (PPARγ), affecting gene transcription involved in glucose and lipid metabolism. It is used to improve glycemic control and has benefits on diabetic dyslipidemia and inflammation[1][3]. The combination aims to leverage the rapid glycemic control achievable through CSII with the persistent metabolic and cardiovascular improvements conferred by metformin and pioglitazone, especially in patients with insulin resistance and inadequately controlled diabetes on oral therapy alone. Fixed-dose combinations of metformin and pioglitazone are available, and coadministration with CSII is considered for patients with difficult-to-manage glycemic profiles, particularly in a hospital or specialized setting[1][3][9].
