Overview

CT-0508 + pembrolizumab is a combination investigational therapy for the treatment of HER2-overexpressing solid tumors. **CT-0508** is a first-in-class autologous monocyte-derived macrophage engineered with an anti-HER2 chimeric antigen receptor (CAR) using an Ad5.F35 adenoviral vector. These macrophages are polarized to an M1 inflammatory phenotype and display potent anti-tumor activity through phagocytosis of HER2-positive tumor cells, tumor microenvironment (TME) remodeling, and activation of T-cell immunity with antigen spreading. **Pembrolizumab** is a checkpoint inhibitor targeting the PD-1 receptor on T cells, blocking interaction with its ligands PD-L1/PD-L2, and thereby releasing inhibition of T-cell activity to promote anti-tumor immunity. The combination is designed to exploit synergistic mechanisms—CT-0508 activates and recruits T cells in the TME, while pembrolizumab sustains T cell responses by inhibiting PD-1 mediated exhaustion. Preclinical and early clinical data show that the combo therapy is well tolerated, increases tumor-infiltrating T and dendritic cells, remodels the TME, enhances antigen presentation, increases T cell clonality, and improves overall tumor control versus either therapy alone in patients with advanced HER2+ solid tumors, including breast and gastroesophageal cancers[1][2][4][5].
