CTX-471 + pembrolizumab

CTX-471 + pembrolizumab is an investigational combination therapy for advanced or metastatic cancers that have progressed while receiving PD-1/PD-L1 inhibitor treatment. CTX-471 is a fully human monoclonal antibody (IgG4) targeting CD137 (4-1BB), acting as an agonist to enhance T-cell and natural killer cell activation. Pembrolizumab is a humanized monoclonal antibody checkpoint inhibitor targeting programmed cell death protein 1 (PD-1), blocking its interaction with PD-L1 and PD-L2 to restore antitumor immune responses. The combination is being evaluated to determine synergistic antitumor efficacy potentially through dual immune activation, though Phase 1 results indicated an unexpected suppression of proinflammatory cytokines not seen with CTX-471 monotherapy. The clinical development of the combination is being discontinued due to this finding. Developed for patients with metastatic or locally advanced malignancies—such as non-small cell lung cancer, head and neck squamous cell carcinoma, melanoma, and others—who have previously failed PD-1/L1 blockade[1][2][3][4].
