Overview

**CVM-1118 + sorafenib is an investigational combination therapy for advanced hepatocellular carcinoma (HCC). CVM-1118 (generic name: foslinanib) is an oral small molecule agent that inhibits vasculogenic mimicry (VM), a process enabling tumor cells to form vascular-like networks, contributing to tumor growth and metastasis[2][3]. Mechanistically, CVM-1118 induces apoptosis, causes G2/M cell cycle arrest, and disrupts VM by targeting TNF receptor-associated protein 1 (TRAP1), reducing HIF-1α stabilization and affecting cellular energetics[2]. Sorafenib is a multi-kinase inhibitor that targets tumor growth and angiogenesis, commonly used as first-line systemic therapy for advanced HCC; however, resistance often develops[3]. CVM-1118’s ability to inhibit VM complements sorafenib’s antiangiogenic action, aiming to overcome sorafenib resistance and improve efficacy in advanced disease[3]. The combination is currently in Phase 2 development for advanced HCC[3].
