This specific cycle consists of a combination of chemotherapeutic agents, including cytarabine (Ara-C), 6-mercaptopurine (6-MP), and pegaspargase (pegylated L-asparaginase). The regimen is designed by the Gruppo Italiano Malattie EMatologiche dell'Adulto (GIMEMA) to achieve early minimal residual disease (MRD) negativity. The combination works through multiple pathways: pegaspargase depletes circulating L-asparagine, an amino acid essential for leukemic cell survival; cytarabine inhibits DNA polymerase and incorporates into DNA to cause chain termination; and mercaptopurine interferes with purine biosynthesis, collectively inducing apoptosis in malignant lymphoblasts.
