Overview

Cyclophosphamide + alemtuzumab (Campath IH) is a combination conditioning regimen used primarily in the setting of allogeneic hematopoietic stem cell transplantation (HSCT) for patients with severe aplastic anemia (SAA). Cyclophosphamide is an alkylating agent that provides potent immunosuppression and myelosuppression by cross-linking DNA. Alemtuzumab is a humanized monoclonal antibody targeting the CD52 antigen, which is highly expressed on T and B lymphocytes, natural killer cells, and monocytes. The combination is designed to achieve profound lymphodepletion and prevent graft rejection and graft-versus-host disease (GVHD) in patients who lack an HLA-identical sibling donor and have failed prior immunosuppressive therapy. This regimen is often used in conjunction with reduced-intensity conditioning, such as low-dose total body irradiation (TBI).
