This experimental combination therapy regimen was developed by the Barbara Ann Karmanos Cancer Institute for the treatment of recurrent or refractory high-grade brain tumors. The protocol utilizes high-dose chemotherapy consisting of cyclophosphamide, cisplatin, and carmustine, supported by autologous bone marrow or peripheral blood stem cell transplantation to mitigate severe hematologic toxicity. Following hematopoietic recovery, patients receive a multi-modal immunotherapy phase comprising an autologous tumor cell vaccine, therapeutic autologous lymphocytes, and aldesleukin (interleukin-2) to stimulate a targeted immune response against residual tumor cells. Cyclophosphamide and carmustine act as DNA alkylating agents, while cisplatin is a platinum-based antineoplastic that forms DNA adducts; together, they aim to overcome chemoresistance in aggressive gliomas like glioblastoma multiforme.
