This is a four-drug combination chemotherapy regimen consisting of cyclophosphamide, doxorubicin, fluorouracil (5-FU), and leucovorin calcium. Cyclophosphamide is an alkylating agent that interferes with DNA replication. Doxorubicin is an anthracycline antibiotic that intercalates DNA and inhibits topoisomerase II, leading to cytotoxicity. Fluorouracil is a pyrimidine analog antimetabolite that inhibits thymidylate synthase, disrupting DNA synthesis. Leucovorin calcium (folinic acid) enhances the binding of 5-FU to thymidylate synthase, increasing its cytotoxic effect. This regimen has been studied primarily in advanced or metastatic breast cancer and other solid tumors such as colorectal cancer[1][3][5]. The addition of leucovorin aims to potentiate the efficacy of 5-FU within the combination[3]. Clinical studies have shown this regimen can be safely administered with significant antitumor activity in breast cancer; however, some trials did not show improved outcomes compared to regimens without leucovorin[1][3].
