CHOPT is a five-drug combination chemotherapy regimen developed and primarily evaluated by researchers at the Tianjin Medical University Cancer Institute and Hospital for the treatment of T-cell lymphomas, specifically peripheral T-cell lymphoma (PTCL). The regimen builds upon the standard CHOP framework—comprising cyclophosphamide, doxorubicin (hydroxydaunorubicin), vincristine (Oncovin), and prednisone—by adding teniposide (VM-26). This addition is intended to enhance the cytotoxic effect against T-cell malignancies, which frequently exhibit resistance to standard anthracycline-based therapies. The components work synergistically: cyclophosphamide acts as a DNA alkylating agent; doxorubicin and teniposide inhibit topoisomerase II to prevent DNA replication; vincristine disrupts microtubule formation to halt mitosis; and prednisone induces apoptosis through glucocorticoid receptor activation. Clinical studies at Tianjin have suggested improved complete response rates and survival outcomes with CHOPT compared to standard CHOP in specific T-cell lymphoma subtypes.
