Overview

A combination investigational regimen consisting of **cyclophosphamide** (an alkylating chemotherapy), **dribble vaccine** (a therapeutic cancer vaccine composed of tumor-derived autophagosomes or lysates, sometimes called "DRibbles"), **sargramostim** (granulocyte-macrophage colony-stimulating factor/GM-CSF, an immune adjuvant and hematopoietic growth factor), and **human papillomavirus (HPV) vaccine** (typically comprised of HPV antigens or peptides, often targeting E6/E7 oncoproteins, for immunotherapeutic purposes). Cyclophosphamide modulates the immune microenvironment, reducing regulatory T cells and myeloid-derived suppressor cells, and can enhance the immune response to vaccines[3]. The dribble vaccine provides tumor antigens in a form that is designed to boost antigen presentation and subsequent T cell activation. Sargramostim (recombinant human GM-CSF) acts as an adjuvant by promoting differentiation and activation of dendritic cells and macrophages, enhancing antigen presentation and immune response[2][4][6]. The HPV vaccine component induces or boosts HPV-specific T cell immunity, most often for treating HPV-associated malignancies (e.g., cervical cancer, head and neck squamous cell carcinoma)[5]. This multi-agent combination is under investigation primarily in the context of immunotherapy for HPV-driven cancers, where synergy between chemotherapy-induced immunomodulation, immune adjuvants, and antigen-specific cellular immune responses are critical for anti-tumor efficacy[3][5].
