This is a combination chemotherapy regimen consisting of four drugs: cyclophosphamide (an alkylating agent), epirubicin (an anthracycline antibiotic), fluorouracil (5-FU, an antimetabolite), and folinic acid (leucovorin, a reduced form of folic acid). The regimen is used primarily as first-line or adjuvant therapy for advanced or high-risk breast cancer. Cyclophosphamide acts by cross-linking DNA strands and inhibiting DNA replication. Epirubicin intercalates into DNA and inhibits topoisomerase II, leading to DNA breaks. Fluorouracil inhibits thymidylate synthase, disrupting DNA synthesis during the S-phase of the cell cycle; its cytotoxicity is enhanced by folinic acid, which stabilizes the binding of 5-FU’s active metabolite to thymidylate synthase. This combination aims to maximize antitumor efficacy through synergistic mechanisms targeting rapidly dividing cancer cells[1][2][7].
