Overview

This is a combination chemotherapy regimen consisting of three cytotoxic agents—cyclophosphamide, epirubicin, and paclitaxel—used primarily in the treatment of breast cancer. Cyclophosphamide is an alkylating agent that interferes with DNA replication and transcription by cross-linking DNA strands. Epirubicin is an anthracycline antibiotic that inhibits topoisomerase II, leading to DNA breaks and apoptosis. Paclitaxel is a taxane that stabilizes microtubules and prevents their depolymerization during cell division, thereby inhibiting mitosis. This combination regimen has been used as neoadjuvant (pre-surgical) or adjuvant (post-surgical) therapy for high-risk early-stage or locally advanced breast cancer—including node-positive disease—and has demonstrated efficacy in improving event-free survival rates compared to conventional regimens[1][3][6][8]. The drugs are typically administered sequentially over several cycles; common protocols include four cycles of epirubicin plus cyclophosphamide followed by four cycles of paclitaxel[6][8]. Toxicities include neutropenia, nausea/vomiting, peripheral neuropathy (from paclitaxel), alopecia, and potential cardiac toxicity (from epirubicin)[1][3][6].
