Cyclophosphamide + epirubicin + thiotepa is a combination chemotherapy regimen used primarily in the treatment of high-risk or metastatic breast cancer. This regimen combines three cytotoxic agents: - Cyclophosphamide, an alkylating agent that crosslinks DNA and inhibits cell division. - Epirubicin, an anthracycline antibiotic that intercalates into DNA and inhibits topoisomerase II, leading to DNA strand breaks. - Thiotepa, another alkylating agent that causes DNA crosslinking and cell death. The combination is often administered as part of high-dose (HD) or tandem HD chemotherapy protocols, sometimes with hematopoietic stem cell support to manage myelosuppression. It has shown particular benefit in patients with BRCA1-like tumor profiles due to their impaired ability to repair double-strand DNA breaks induced by these agents[1][2]. The regimen aims for maximal cytotoxic effect against rapidly dividing tumor cells.
