Cyclophosphamide + fludarabine + antithymocyte globulin (CyFluATG) is a conditioning regimen developed by Asan Medical Center for patients with lower-risk myelodysplastic syndromes (MDS) undergoing allogeneic hematopoietic cell transplantation (HCT). This regimen combines two chemotherapy agents—cyclophosphamide, an alkylating agent, and fludarabine, a purine analog—with antithymocyte globulin (ATG), a polyclonal antibody. The protocol is designed to provide sufficient immunoablation and myelosuppression to allow for donor cell engraftment while minimizing non-relapse mortality and the risk of graft-versus-host disease (GVHD). It is currently being evaluated in a Phase 2 extension study to assess its feasibility and efficacy in the lower-risk MDS population.
