This combination therapy consists of cyclophosphamide, an alkylating agent and potent immunosuppressant, and glucocorticoids (such as prednisone or prednisolone), which are corticosteroid anti-inflammatory medications. It is being evaluated by Peking Union Medical College Hospital in a Phase 4 clinical trial for the treatment of idiopathic retroperitoneal fibrosis (IRPF), a rare fibroinflammatory disease. Cyclophosphamide works by cross-linking DNA, thereby inhibiting cell division and suppressing the immune response, while glucocorticoids provide rapid anti-inflammatory effects. The regimen typically involves a tapering dose of steroids combined with a daily or every-other-day dose of cyclophosphamide to induce and maintain remission of the fibrotic process.
