Overview

This is a multi-agent Graft-versus-Host Disease (GVHD) prophylaxis regimen evaluated by the Medical College of Wisconsin in the Phase II BMT CTN 0603 trial for patients with leukemia or lymphoma undergoing haploidentical bone marrow transplantation. The regimen consists of high-dose post-transplant cyclophosphamide (PTCy) administered on days 3 and 4, followed by tacrolimus, mycophenolate mofetil (MMF), and granulocyte colony-stimulating factor (G-CSF) starting on day 5. PTCy works by selectively eliminating alloreactive T cells while preserving regulatory T cells and hematopoietic stem cells. Tacrolimus and MMF provide additional immunosuppression by inhibiting calcineurin and inosine monophosphate dehydrogenase, respectively, while G-CSF is used to support myeloid recovery and neutrophil engraftment.
