Overview

Cyclophosphamide + thiotepa is a combination of two alkylating agents used primarily as a high-dose conditioning regimen before hematopoietic stem cell transplantation (HSCT) and in the treatment of certain malignancies. Cyclophosphamide is an oxazaphosphorine alkylating agent that interferes with DNA replication and RNA transcription, leading to cell death. Thiotepa is a polyfunctional alkylating agent that cross-links DNA strands, inhibiting DNA synthesis and function. The combination exploits their non-overlapping toxicities and different mechanisms to maximize cytotoxicity while minimizing resistance. This regimen has been studied as an alternative to total body irradiation (TBI)-based conditioning for acute lymphoblastic leukemia (ALL) patients undergoing myeloablative allogeneic stem cell transplantation, showing similar efficacy but reduced toxicity such as lower rates of mucositis, bacterial infection, sinusoidal obstruction syndrome (SOS), and shorter hospital stays[1][2]. It has also been explored in autologous bone marrow transplantation settings for solid tumors[2].
