This combination regimen is a conditioning protocol used prior to allogeneic hematopoietic stem cell transplantation (HSCT) in pediatric patients with high-risk hematologic malignancies. Developed and utilized by St. Jude Children's Research Hospital, the regimen consists of chemotherapy (cyclophosphamide and thiotepa), an antibody (anti-thymocyte globulin, ATG), and total body irradiation (TBI). The primary goal of this combination is to achieve myeloablation and profound immunosuppression to facilitate donor cell engraftment and prevent graft rejection. Cyclophosphamide and thiotepa act as DNA alkylating agents to eradicate residual leukemic cells, while ATG provides T-cell depletion to reduce the risk of graft-versus-host disease (GVHD). TBI serves as both a myeloablative and an antineoplastic component, ensuring the clearance of the recipient's bone marrow space.
