Cyclophosphamide + veliparib is an investigational combination therapy consisting of the alkylating agent cyclophosphamide and the poly(ADP-ribose) polymerase (PARP) inhibitor veliparib. Cyclophosphamide induces DNA crosslinking and damage, leading to cell death, while veliparib inhibits PARP enzymes involved in DNA repair, thereby potentiating the cytotoxic effects of DNA-damaging agents like cyclophosphamide. This combination has been studied primarily in patients with refractory solid tumors and lymphoid malignancies, as well as BRCA-mutant ovarian cancer and triple-negative breast cancer. The rationale for combining these agents is that PARP inhibition impairs tumor cells' ability to repair DNA damage caused by cyclophosphamide, resulting in increased tumor cell death[1][2][6]. Clinical trials have shown that this regimen is generally well tolerated and may be particularly effective in patients with BRCA mutations.
