Regimen 2, Course 2 is a specific multi-agent chemotherapy protocol developed by Baylor College of Medicine for the treatment of brain tumors in infants and young children (typically under 3 years of age). This regimen is part of a pilot clinical study designed to evaluate the safety and efficacy of intensive systemic chemotherapy combined with intrathecal mafosfamide and conformal radiation, aiming to delay or avoid the need for high-dose craniospinal irradiation. Regimen 2, Course 2 is a repetition of Course 1, which consists of cycles of intravenous cyclophosphamide (a DNA alkylating agent), vincristine (a microtubule inhibitor), and oral etoposide (a topoisomerase II inhibitor). Mesna is administered concurrently with cyclophosphamide as a uroprotective agent to prevent hemorrhagic cystitis. The protocol is specifically designed to manage pediatric central nervous system malignancies while minimizing long-term neurocognitive side effects associated with standard radiation therapy in very young patients.
