Overview

A combination therapy consisting of the calcineurin inhibitor cyclosporin and the folate antagonist methotrexate, investigated by Georgetown University for the treatment of newly diagnosed Type 1 Diabetes Mellitus (T1DM). Cyclosporin acts as an immunosuppressant by binding to cyclophilin and inhibiting calcineurin, which prevents the dephosphorylation of the nuclear factor of activated T-cells (NFAT) and subsequent T-cell activation. Methotrexate, used at low doses in this context, inhibits dihydrofolate reductase and promotes the release of adenosine, exerting anti-inflammatory and immunomodulatory effects. The regimen was evaluated in a Phase 2 pilot study (NCT00004457) to determine if suppressing the autoimmune response could preserve pancreatic beta-cell function and induce clinical remission in pediatric patients.
