Cysteamine + isobionicamide is a topical combination therapy being evaluated for the treatment of pigmentary skin disorders, specifically post-inflammatory hyperpigmentation (PIH). Cysteamine is a naturally occurring small molecule aminothiol that acts as a potent antioxidant and depigmenting agent. Its mechanism involves the inhibition of tyrosinase and peroxidase enzymes, which are critical in the melanin synthesis pathway, as well as the elevation of intracellular glutathione levels. Isobionicamide is included in the complex to potentially enhance the stability or efficacy of the formulation. This specific drug complex was the subject of a randomized, double-blind, vehicle-controlled Phase 2 clinical trial (NCT05206318) conducted by Chang Gung Memorial Hospital, which investigated its safety and efficacy in reducing the melanin index in patients with PIH.
