**D-tagatose + metformin** is a proposed combination therapy for type 2 diabetes, pairing the rare sugar D-tagatose, a low-calorie hexose with poor digestibility, and metformin, the standard first-line biguanide. D-tagatose acts via a unique mechanism by being minimally absorbed in the small intestine (~25%), fermented by gut microbiota to produce short-chain fatty acids, thereby blunting postprandial glucose excursions, lowering fasting blood glucose and HbA1c, and improving lipid profiles (e.g., reducing LDL/total cholesterol) without promoting weight gain or hypoglycemia. Metformin decreases hepatic gluconeogenesis, enhances insulin sensitivity, and reduces intestinal glucose absorption. The combination is theorized for additive/synergistic glycemic control given D-tagatose's distinct mode orthogonal to metformin's, potentially addressing unmet needs in patients with suboptimal control on metformin monotherapy; however, no dedicated clinical trials for this exact pairing have been identified, unlike D-tagatose monotherapy (phase 3 completed) or other combos like SPX-106T.
