D-TRANS fentanyl + naltrexone is an abuse-deterrent transdermal drug delivery system developed by ALZA Corporation (a subsidiary of Johnson & Johnson) for the management of chronic pain in opioid-tolerant patients. The system utilizes ALZA's proprietary D-TRANS technology to deliver fentanyl, a potent mu-opioid receptor agonist, while sequestering naltrexone, a mu-opioid receptor antagonist. The patch is designed so that naltrexone does not significantly permeate intact skin, allowing the therapeutic delivery of fentanyl for analgesia. However, if the patch is tampered with—such as through extraction for oral or intravenous use—the naltrexone is released to antagonize the opioid effects of fentanyl, thereby deterring misuse and abuse. Clinical development reached at least Phase 2 safety evaluations comparing the system to standard fentanyl patches like Duragesic.
