D2C7 CARE-BEAR refers to a combination therapeutic regimen consisting of D2C7-IT, a recombinant immunotoxin, and atezolizumab, a PD-L1-targeted checkpoint inhibitor. D2C7-IT is a fusion protein comprising a monoclonal antibody (D2C7) that dual-targets both wild-type epidermal growth factor receptor (EGFR) and its variant III (EGFRvIII), conjugated to a truncated Pseudomonas exotoxin (PE38). This combination is primarily investigated for the treatment of recurrent glioblastoma and gliosarcoma. The mechanism of action involves D2C7-IT-mediated direct cytotoxicity through the inhibition of protein synthesis in tumor cells, while atezolizumab enhances the anti-tumor immune response by blocking the interaction between PD-L1 and its receptors, PD-1 and B7-1. The regimen utilizes convection-enhanced delivery (CED) for the localized administration of D2C7-IT directly into the tumor or resection cavity, combined with systemic intravenous administration of atezolizumab.
