Dacarbazine + interferon alfa + thymalfasin is a combination immunochemotherapy regimen investigated for the treatment of stage IV metastatic malignant melanoma. The regimen consists of dacarbazine (DTIC), a small-molecule alkylating agent; interferon alfa (IFN-α), a cytokine biologic; and thymalfasin (thymosin alpha-1), a synthetic polypeptide immunomodulator. Developed by Sigma-Tau (now Leadiant Biosciences), the combination aims to leverage the cytotoxic effects of dacarbazine alongside the immunostimulatory properties of interferon and thymalfasin. Thymalfasin enhances T-cell maturation and effector functions, potentially overcoming the immunosuppressive environment of advanced melanoma. A Phase II multicenter study (NCT00911443) demonstrated that the addition of thymalfasin to the DTIC and IFN-α backbone was well-tolerated and suggested improved durable response rates and overall survival compared to standard treatments.
