Overview

Daclizumab + mycophenolate mofetil is a combination immunosuppressive regimen used primarily in the context of solid organ transplantation and has also been studied in autoimmune diseases such as type 1 diabetes. Daclizumab is a humanized monoclonal antibody that targets CD25 (the alpha subunit of the interleukin-2 receptor) on activated T lymphocytes, thereby inhibiting IL-2 mediated activation and proliferation of T cells. Mycophenolate mofetil is a prodrug of mycophenolic acid that acts as a selective inhibitor of inosine monophosphate dehydrogenase (IMPDH), an enzyme critical for de novo guanine nucleotide synthesis in lymphocytes; this results in suppression of both T and B cell proliferation. The combination aims to provide potent immunosuppression while minimizing nephrotoxicity compared to calcineurin inhibitors. This regimen has been shown to reduce acute rejection rates after transplantation but did not demonstrate efficacy for preserving beta-cell function in new-onset type 1 diabetes[1][2][5][7].
