Dalpiciclib + endocrine therapy is a combination regimen used for the treatment of hormone receptor (HR)-positive, human epidermal growth factor receptor 2 (HER2)-negative advanced or metastatic breast cancer. Dalpiciclib is a novel, orally administered small molecule inhibitor of cyclin-dependent kinases 4 and 6 (CDK4/6), which are key regulators of cell cycle progression. Endocrine therapies in this context include aromatase inhibitors such as letrozole or anastrozole, or the estrogen receptor degrader fulvestrant. The combination works by inhibiting tumor cell proliferation through CDK4/6 blockade while simultaneously suppressing estrogen-driven signaling pathways that fuel HR-positive breast cancer growth. Clinical trials have demonstrated that this combination significantly improves progression-free survival and objective response rates compared to endocrine therapy alone in both first-line and previously treated settings[1][2][3][4][5][6][7].
