CL4 is a combination neoadjuvant therapy regimen being evaluated in the FUTURE-NEO umbrella study (NCT04632992) for patients with HR-positive, HER2-negative breast cancer. The regimen is composed of dalpiciclib (SHR6390), a selective small-molecule inhibitor of cyclin-dependent kinases 4 and 6 (CDK4/6); letrozole, an aromatase inhibitor that blocks estrogen synthesis; and adebrelimab (SHR-1316), a humanized monoclonal antibody targeting programmed death-ligand 1 (PD-L1). For premenopausal patients, the regimen also includes goserelin, a gonadotropin-releasing hormone (GnRH) agonist used for ovarian suppression. This multi-modal approach aims to overcome endocrine resistance and enhance anti-tumor efficacy by simultaneously targeting the cell cycle, hormonal signaling, and immune evasion pathways.
