A combination neoadjuvant therapy regimen consisting of dalpiciclib, letrozole, trastuzumab, and pertuzumab. Dalpiciclib is a selective small-molecule inhibitor of cyclin-dependent kinases 4 and 6 (CDK4/6), which are key regulators of the cell cycle; its inhibition leads to G1 phase arrest and prevents the proliferation of cancer cells. Letrozole is a non-steroidal aromatase inhibitor that suppresses systemic estrogen production, thereby depriving hormone receptor-positive (HR+) breast cancer cells of growth signals. Trastuzumab and pertuzumab are monoclonal antibodies that target different epitopes of the human epidermal growth factor receptor 2 (HER2), providing a comprehensive dual blockade of HER2-mediated signaling pathways. This specific combination is being investigated in a Phase 2 study by Henan Cancer Hospital as a potential chemotherapy-free neoadjuvant treatment for patients with hormone receptor-positive (HR+) and human epidermal growth factor receptor 2-positive (HER2+) early or locally advanced breast cancer.
