Overview

This combination therapy consists of the oral sodium-glucose cotransporter 2 (SGLT2) inhibitor dapagliflozin and the intravitreal vascular endothelial growth factor (VEGF) inhibitor aflibercept. It is being investigated by Alexandria University as a treatment for center-involved diabetic macular edema (DME). Dapagliflozin, typically used for glycemic control in type 2 diabetes, is hypothesized to act as an adjunct to standard-of-care anti-VEGF therapy by further reducing retinal thickness through systemic glucose lowering and potential local osmotic or anti-inflammatory effects. Aflibercept works by binding to VEGF-A, VEGF-B, and placental growth factor (PlGF), thereby inhibiting the signaling pathways that lead to vascular leakage and macular swelling.
