Dapagliflozin + finerenone is a combination regimen consisting of a sodium-glucose cotransporter-2 (SGLT2) inhibitor and a non-steroidal mineralocorticoid receptor antagonist (MRA). Dapagliflozin works by inhibiting glucose reabsorption in the proximal tubule of the kidney, leading to glucosuria and osmotic diuresis, which reduces intraglomerular pressure. Finerenone selectively blocks the mineralocorticoid receptor, thereby inhibiting inflammation and fibrosis in the heart and kidneys. This combination is being investigated for its potential additive or synergistic effects in reducing albuminuria and slowing the progression of chronic kidney disease in patients with type 2 diabetes (diabetic nephropathy). The regimen is currently being evaluated in investigator-initiated clinical trials to determine if the dual blockade of these pathways provides superior renoprotection compared to monotherapy.
