The DAPT regimen combined with radiotherapy is an investigational therapeutic protocol developed by Peking University Cancer Hospital & Institute for the treatment of stage I/II extranodal NK/T-cell lymphoma (ENKTL). This multi-modal approach integrates the PD-1 inhibitor tislelizumab, the hypomethylating agent azacitidine, the asparagine-depleting enzyme pegaspargase, and the corticosteroid dexamethasone, followed by or concurrent with radiotherapy. The regimen aims to overcome the aggressive nature of ENKTL by combining immune checkpoint blockade with epigenetic modulation and metabolic disruption. Tislelizumab enhances the anti-tumor T-cell response, while azacitidine targets DNA methylation patterns often dysregulated in EBV-associated lymphomas. Pegaspargase exploits the asparagine dependence of NK/T-cell lymphoma cells, and radiotherapy provides essential local control for early-stage disease.
