Daptomycin + meropenem is a combination of two intravenous antibiotics used off-label for the treatment of severe or complicated infections, particularly when broad-spectrum coverage is required. Daptomycin is a cyclic lipopeptide antibiotic that exerts rapid, concentration-dependent bactericidal activity against Gram-positive bacteria by binding to bacterial membranes in a calcium-dependent manner and causing membrane depolarization and cell death. Meropenem is a carbapenem beta-lactam antibiotic with potent bactericidal activity against many Gram-negative and some Gram-positive organisms; it acts by inhibiting bacterial cell wall synthesis through high-affinity binding to penicillin-binding proteins (PBPs), especially PBPs 2, 3, and 4[5]. This combination may be considered in cases such as severe cellulitis or suspected necrotizing fasciitis where both Gram-positive (including MRSA) and resistant Gram-negative pathogens are possible[2].
