Overview

DCd is a combination chemotherapy and immunotherapy regimen consisting of daratumumab, cyclophosphamide, and dexamethasone. It is primarily used for the treatment of patients with relapsed and refractory multiple myeloma (RRMM), particularly those who are lenalidomide-refractory. Daratumumab is a human IgG1 monoclonal antibody that targets CD38, a glycoprotein highly expressed on myeloma cells, inducing cell death through multiple immune-mediated mechanisms including antibody-dependent cellular cytotoxicity (ADCC) and complement-dependent cytotoxicity (CDC). Cyclophosphamide is an alkylating agent that interferes with DNA replication and transcription, while dexamethasone is a corticosteroid that provides anti-inflammatory and immunosuppressive effects while enhancing the efficacy of other myeloma therapies. The regimen was evaluated in a Phase II trial (NCT02951819) by the Canadian Myeloma Research Group (CMRG) as a backbone for RRMM treatment.
