daratumumab + pomalidomide + dexamethasone + all-trans retinoic acid is a combination therapy regimen being evaluated by Hackensack Meridian Health for the treatment of relapsed or refractory multiple myeloma. The regimen is specifically designed for patients who have previously been exposed to daratumumab-based therapies and may have developed resistance. The therapeutic rationale for adding all-trans retinoic acid (ATRA) is its ability to upregulate the expression of CD38 on the surface of multiple myeloma cells, which often decreases during daratumumab treatment. By increasing CD38 density, ATRA aims to restore or enhance the efficacy of daratumumab, an anti-CD38 monoclonal antibody that mediates tumor cell death through antibody-dependent cellular cytotoxicity (ADCC) and complement-dependent cytotoxicity (CDC). This is combined with pomalidomide, an immunomodulatory drug (IMiD) that induces the degradation of transcription factors Ikaros and Aiolos via the cereblon E3 ubiquitin ligase complex, and dexamethasone, a corticosteroid that provides synergistic anti-tumor and anti-inflammatory effects.
