**darbepoetin alfa + iron dextran** is a combination therapy consisting of a recombinant erythropoiesis-stimulating agent (ESA) and an intravenous iron replacement product. - **Darbepoetin alfa** is a modified erythropoietin with increased sialic acid content and a longer serum half-life than endogenous erythropoietin, stimulating erythropoiesis by acting as an agonist of the erythropoietin receptor on erythroid progenitor cells, enhancing red blood cell production[2][6]. - **Iron dextran** is a colloidal solution of ferric oxyhydroxide complexed with dextran, used intravenously to replenish iron stores and correct iron deficiency anemia by increasing available iron for hemoglobin synthesis[4]. This combination is primarily indicated for treatment of anemia associated with chronic kidney disease and chemotherapy-induced anemia in patients unresponsive to ESA monotherapy due to functional iron deficiency. Clinical studies demonstrate that supplementing darbepoetin alfa with intravenous iron dextran significantly improves hemoglobin response in patients with inadequate erythropoietic response, as iron availability is often a limiting factor for ESA effectiveness[3].
