Darunavir + cobicistat + lamivudine is a three-drug combination used in the treatment of HIV-1 infection. Darunavir is a protease inhibitor that prevents HIV replication by binding to and inhibiting the viral protease enzyme, thereby blocking the maturation of infectious virus particles. Cobicistat acts as a pharmacokinetic enhancer (booster) by inhibiting cytochrome P450 3A (CYP3A) enzymes, which increases plasma concentrations of darunavir, allowing for once-daily dosing and improved efficacy. Lamivudine is a nucleoside reverse transcriptase inhibitor (NRTI) that inhibits reverse transcriptase, an essential enzyme for viral replication in HIV. This combination regimen offers an alternative to traditional three-drug regimens and may be considered when other backbones such as emtricitabine/tenofovir cannot be used[1][2][9]. The combination aims to maintain virologic suppression while potentially reducing toxicity and cost.
