Overview

This is a combination of three antiretroviral drugs used in the treatment of HIV-1 infection. Darunavir is a protease inhibitor that prevents viral replication by inhibiting the HIV-1 protease enzyme, which is essential for processing viral proteins and maturation of infectious virus particles. Ritonavir acts primarily as a pharmacokinetic enhancer (booster) by inhibiting cytochrome P450 3A4 (CYP3A4), thereby increasing plasma concentrations of darunavir and improving its efficacy. Rilpivirine is a non-nucleoside reverse transcriptase inhibitor (NNRTI) that blocks HIV-1 replication by binding non-competitively to reverse transcriptase, preventing RNA and DNA-dependent DNA polymerization. This combination regimen may be used as an NRTI-sparing option for patients with virologically suppressed HIV or those experiencing toxicity from nucleoside/nucleotide reverse transcriptase inhibitors (NRTIs). The combination has demonstrated high efficacy, good tolerability, and favorable safety profiles in clinical studies[2][3][4][5].
