Overview

The combination of decitabine and penpulimab is an emerging therapeutic approach in oncology. Decitabine is a hypomethylating agent (HMA) that works by inhibiting DNA methyltransferase, leading to DNA hypomethylation and reactivation of silenced genes. Penpulimab is an anti-PD-1 IgG1 antibody designed to reduce immune-related adverse events while maintaining efficacy. The combination leverages complementary mechanisms: Decitabine can enhance the efficacy of immune checkpoint inhibitors like penpulimab by increasing CD8+ T cell infiltration into tumors, reversing T-cell exhaustion, and potentially helping to overcome resistance to PD-1 inhibitors. While specific clinical trial results for this combination were not found in the search data, similar combinations of hypomethylating agents with PD-1 inhibitors have shown promising results in other studies, such as low-dose decitabine with camrelizumab in advanced non-small cell lung cancer and guadecitabine with pembrolizumab in a Phase 1 study. Individually, penpulimab is FDA-approved for certain indications in nasopharyngeal carcinoma, and decitabine is FDA-approved for myelodysplastic syndrome and acute myeloid leukemia. Decitabine is also available in an oral formulation (decitabine/cedazuridine) and is being investigated in various other combinations.
