This combination therapy consists of the iron chelator deferasirox and a prebiotic, potato resistant starch (PRS), being investigated by the University of Michigan Rogel Cancer Center. The regimen is designed to prevent graft-versus-host disease (GVHD) in patients undergoing allogeneic hematopoietic stem cell transplantation (alloHCT). Deferasirox reduces systemic and local iron levels, which are often elevated in transplant patients and can promote oxidative stress and inflammation. Potato resistant starch is a fermentable fiber that reaches the large intestine, where it is metabolized by gut microbiota into short-chain fatty acids (SCFAs) like butyrate. These SCFAs help maintain intestinal barrier integrity and exert anti-inflammatory effects, potentially synergizing with iron chelation to mitigate the risk of GVHD by modulating both the host environment and the gut microbiome.
