Overview

This therapeutic regimen is a combination therapy consisting of two iron chelating agents, deferoxamine and deferiprone, administered in conjunction with regular blood transfusions for the treatment of patients with transfusion-dependent beta-thalassemia. Deferoxamine is a parenteral hexadentate iron chelator that binds ferric iron to form ferrioxamine, which is excreted by the kidneys and in the feces. Deferiprone is an orally active bidentate iron chelator that forms a 3:1 (chelator:iron) complex, facilitating iron excretion primarily through the urine. The combination of these two agents, often referred to as dual chelation therapy, is employed to achieve a synergistic effect in reducing systemic iron overload, particularly in the heart and liver, which is a common complication of chronic blood transfusion therapy. This specific protocol was evaluated by Ain Shams University in a clinical trial focusing on patients with low serum ferritin levels to determine the safety and efficacy of continuing chelation therapy versus discontinuation.
